We used to get a Medical Affirmation form from the County every November, the one where we had to swear that the people listed as dependent were still eligible. I didn't get it this year. Has anyone else gotten it?

Wiz, I only got one form last year and I have not gotten anything else since. I thought it was a one time thing to make sure that your kids under 26 or so were still your dependents living at home. I believe it was part of the obama health care law. I'll let you know if I get anything else in the mail about this. I'm still waiting to hear what my new Social Security monthly check will be with the new Medicare cost taken out of it.By the way, my four kids are in their late forties to early fifties and on their own.

I've gotten it every year since I retired. Every year they gave me less and less time to return it. This is the first year I have a change to make. My youngest is in the Air Force and no longer eligible. I don't know who to call to have him removed. I was waiting for that form to do it.

Wizard. I keep a copy of every thing NY sends to me. If you are referring to the NYSHIP Dependent Eligibility Project, their phone number is 1-888-358-2198 address PO Box 8072 Royal Oak MI 48068. I hope this helps you.

Thanks NCPDRetired but this thing came direct from the County. Maybe the people who used to do it got laid off. That would be par for the course for the County.

I went back and checked all my paper work and I never got anything from the county. Maybe because I am a tier 1 guy? All I ever got was paper from the State & Civil Service & NYSHIP.

I remember something from the County. I don't remember if it was every year though. I could be confusing it with the one from NYSHIP.

Thanks Redstone. That number is a recording at the Controller's Office. I think that's where the forms used to come from but I don't talk to machines.
NCPDRETIRED, I'm Tier 1 so that's not the problem. It may be because I had a minor child as a dependent.

Son of a gun! It came today. It's a month late but it came, typical for Nassau County. The title of this thing is the "Enrollee's 2012 Annual Attestation Letter" and it goes to the Controllers Office. Am I the only guy who still gets these things?
At least it didn't come with a bill.

Wiz. Happy you got the form that you were worried about. Down here in Sunny Florida. have not received it as of yet,but then again its just me and the Wife on my hospitalization.
